Robert Hazelwood, MD

  Robert Hazelwood, MD, 73, of Sioux Falls, SD, passed away on Monday, July 24, 2023 at his residence in Tea, SD. A Celebration of Life will be held at 5:00 p.m. on Friday, August 4, 2023 at Harvest Church in Sioux Falls. Private interment has taken place in Calvary Cemetery.

  Robert was born on December 15, 1949 in Springfield, Illinois, to Harold and Christina (Williamson) Hazelwood.

  On June 21, 1997 he married Vickie Sampson Crawford in Terre Haute, Indiana.

  Survivors include his wife, Vickie Hazelwood; his children: Ashley Allen (Brian) of Middleburg, FL, and Evan Hazelwood of Terre Haute, IN; brother, James Hazelwood of Springfield, IL; Vickie's sons: Adam Crawford of Sioux City and Lee Crawford of Sheldon, IA; five grandchildren: Madison, Hayden, Zachary, Paige, and Avery.

  He was preceded in death by his parents.
