Phyllis Ann (Lohse) Hayworth, age 89, of Sioux City, Iowa, died April 4, 2026, at home. A Memorial Service will be held at 11:00 a.m. Wednesday, April 29, 2026 at Sunnybrook Community Church with a visitation one hour prior to service time. Burial will follow at Westfork Township/Climbing Hill Cemetery.
Phyllis was born in 1936 in Sioux City, Iowa, to John and Hazel (Spangler) Lohse and was the oldest of three children, two girls (Phyllis and Norma) and a boy (John Jr). She attended Moville Elementary School and graduated from Climbing Hill High School.
In 1956, she married Frank Hayworth of Anthon, and after Frank concluded a brief military tenure, the young couple transitioned to farm life near Anthon. During time on the farm, she gave birth to her first son, Brad, in 1957. The family then moved to Wayne, Nebraska, and Phyllis worked to support her husband’s college education. While in Wayne, her second son, Kurt, was born in 1960.
Post-college employment took the family to Omaha, Nebraska, for many years where Phyllis remained a dedicated wife and mother. Her work efforts again were crucial to Frank’s career path including medical school. Her love of family brought another child, daughter Angela, to the family in 1973.
After further life changes, Phyllis attended and graduated with highest honors from the College of St Mary in Omaha. Upon graduation, she began a career in medical records administration and held positions at hospitals in Kentucky and Iowa, while still fulfilling the role of mother. After her medical-related career, she taught at Western Iowa Technical Community College in Sioux City for several years.
Phyllis enjoyed traveling across the country and the world to see her children, grandchildren, and great-grandchildren, and she maintained meaningful relationships with friends both near and far. She was an avid reader, energetic sports fan, skilled gardener, superb cook, and creative seamstress. Pets were always welcome residents in her home. Her interest in the British royal family was extraordinary. In latter years, she found the Texas Gulf Coast as a suitable alternative to Iowa winters. She retained Christian convictions throughout her life and had a lively patriotic spirit.
Phyllis had four grandchildren (Kathryn, Holly, Laura, Daniel) and seven great-grandchildren (Ronald, Vincent, Jade, Jordan, Ivy, Joshua, Jasper). She is survived by her three children, a beloved daughter-in-law Kim, her sister Norma and brother-in-law Nick Kohlhof, as well as many nieces and nephews. She was preceded in death by her parents and brother.
She will be greatly missed and long remembered for her beautiful, fruitful life of loving sacrifice, warm hospitality, and pleasant strength.
